Per $1000 of coverage
<25 $1.20
25-29 $0.96
30-34 $1.20
35-39 $1.44
40-44 $1.80
45-49 $2.76
50-54 $4.68
55-59 $8.64
60-64 $13.08
65-69 $19.68
70+ $46.68

What I want to be able to do is the following

DOB Age Rate
09/17/1943 ? $13.08/1000

I want Excel to be able to take the birth date calucalte the age then find
the coresponding rate.
