Default How can I create a template that allows me to track patients?

I need a way to track patients by name, SS#, etc., (using columns), that will
allow me to constantly update/edit the template while tracking abnormal lab
results--I need also to be alerted when a patient did not keep a scheduled
appointment (by a red flag or something). Help!!
