Default Designing Form

I need to design a form used for registering swimmers for a team that can
import data from Excel or Access. Can a Word form be used?

Default Designing Form

Both Access and Excel have their own forms. Look at Excel Help and search
for the term Data Form. They're pretty easy to build and use.
