Default Form image

I am trying to create a form in Excel where the data is computed using
the functionality of Excel but the spreadsheet contains an image of a
form. Think along the lines of a tax return where the actual line
numbers are computed from Excel but the form data is overlaid. Is it
possible to do this and if so, how?
