Default how to request Code-signing certificate

I'm not really sure how you would generate the request. Maybe through the
Certificates snap-in to the MMC. Go to start | run and type mmc and hit
enter. Then add the certificates snap-in. Right-click on Personal and
select All Tasks -- Request New Certificate and go from there. Does that
work, or is that what you've already tried?

I know IIS can generate certificate requests in the format your CA is
looking for - maybe you could go that route? Sorry - I've never done
code-signing with anything besides the selfcert.exe option.
