Default How do I create a "Yes / No tick box"?

I want to have a spreadsheet with a list of expected actions.

I want the user to simply tick Yes to indicate that it succeeded and No
to indicate that it failed.

Any ideas?

You could create a check box from the Forms (Or "Control Toolbox")
Toolbar.
Go to ViewToolsbarsForms and insert the checkbox.
