hyperlink with vlookup

=HYPERLINK[=vlookup(a2;CARATULAS!$A$4:$D$146;4;0)]

IÂ´ve got to create an hyperlink that (according to a value in the same row)
takes me to the same value in another worksheet.

Please help me!-
