How do I create a data input form in 2007 like I did in 2003?

You can add this option to the QAT.
It is not in the Ribbon

Default Form Creation in Excel 2007

You need to have the Developer tab show. I believe this is accomplished by
clicking the office icon in the upper left hand corner and configurng XL
options to show it.
