Default HLOOKUP using multiple columns

I have a data sheet that I am trying to put a weight factor at the end of
each row depending on 3 variable (3 columns) Is there a way using HLOOKUP to
match data in muliple columns.

Ex.
Column 1 Column 2 Column 3
# of Employees Industry Code Payroll Frequency

I have about 128 different weight factors depending on the different number
of combinations the data in the above 3 columns contains.

Any help would be greatly appreciated!!!
