With your sample data in cells A2:C6, the desired Card# in A9, and the
desired Package code in B9, use this array formula (press Ctrl-Shift-Enter to
commit it)

=INDEX(C2:C6,MATCH(A9&B9,A2:A6&B2:B6,0))
