Default How do I create startup boxes that once answered appear in a form

I have a form that I want staff to populate pop up questions when they open:
e.g. Todays date, job number

Dont know what this is called to be able to do a search.

I also have the e.g. date appearing in three places on the form - is this
possible instead of having to answer the question three times
