Default Form drop down list

How do you populate multiple fields, based on user selection from a single
drop down list, using word or excel (Office 2003)?

Ok, I used this sample and the sample for vlookup and got everything working
in excel, but then spoke got clarification on the requested form and it needs
to be in word (because the form can potentially be very lengthy). How can I
get the same functionality in word? Thank you.
