Default Macro and Macro Button

I want to be able to create a macro that allows the end user to add a row
anywhere in the worksheet. If they enter something in that row, I want the
previous row contents deleted - beginning with Column C. Is it possible to
add a radio button to run the macro?

Beginning data:
Drug Name Retail Mail
Boniva $85.00
Lipitor $20.00
Levothyroxine $7.00

I want the data to look like:

Drug Name Retail Mail
Boniva $85.00
Lipitor
Simvastatin 10.00
Levothyroxine $7.00

Any help would be appreciated!
