Default forms toolbar greyed out

I have Excel 2003. I am trying to create a command button on a screen. I
select ViewToolbarsForm and the toolbar appears but is greyed out. It
won't allow me to left click the command button on the toolbar and create it
on the screen.

I can open a new notebook and create the command button there but not on the
existing notebook. I have tried to copy a command button from the new
notebook to the existing notebook and receive a pop up stating "Microsoft
Excel cannot paste the data".

The other toolbars are available and not greyed out.

Any suggestions?
