Default Form Question

I need to create a form to do the following.

I need a user to see values from WS5.Range("A1:A1500") no blank values
included
and to be able to select multiple values and then use the values to filter
WS4 by Column 3/or C for each value and then go to the next on the list and
keep going, until the end.

I also need a check box on the form to print WS1 if checked.
