Default How to create drill down scope form?


I NEED TO CREATE A SCOPE DEFINITION FORM USING EITHER AN EXCEL
SPREADSHEET OR VBA WHICH WILL REQUIRE ADDITIONAL INFORMATION IF "YES"
IS SELECTED BUT MOVES ONTO NEXT QUESTION IF NO IS SELECTED AND ALLOWS
TEST TO BE SUMMARIZED AS RESPONSES TO "YES" ANSWERS.
