Depends on what you have to start with, are you starting from your windows address list.
In which case you would export as csv file with the field that you want. As to formatting the list, if for personal use containing friends or club members, I'd suggest something like the following columns in this order: cellphone: Phone: Lastname: firstname: Address: ST (state): zip+4: email: This way the phone numbe is to the immediate left, and the address is to the immediate right which helps you sight down to where you want to be.
