Default printing onto re printed form

I've done this by trial and error. I made a handful of photocopies of
the form in question, then started placing form entries on a
spreadsheet by guessing approximately where they should go- then print,
adjust, print, adjust, print, adjust until the fields are in the proper
location.

Hopefully you're not dealing with carbon copy forms- if you are you'll
need an impact printer or dot matrix printer.
