Default Forms

ARGH! I don't get it.

I have a table of data. I'd like to make a form on another sheet showing
the fields already populated but then add additional fields. I could use the
simple form but I want to resize it and I can't figure out how to do that.
Does anyone know how to resize a simple form?

I'd also like to make some of the fields a checkbox or a yes/no form field;
is that possible to do on a simple form? Any help you can give me is much
appreciated.
