Default How to make a address label ?

Can someone help me to make address label where my excel spreed sheet is like
this :

No Name ID Email Phone1 Phone2 address1 address2 address3

i want it to be display in word or excel as at below :

NAME ID
ADDRESS1 ADDRESS1
ADDRESS1
ADDRESS2 ADDRESS2

Default How to make a address label ?

The best way is to use MS Word's mail merge facility, and tell it to use
your Excel file as the source data. Word help will tell you how.
