In excel 2003 I can add a customized command to the top of the tool bar, but
when I close out of Excel, and re-open it goes away. Any Ideas?

Customize your toolbar in code.

For additions to the worksheet menu bar, I really like the way John Walkenbach
does it in his menumaker workbook:
