Default CAN YOU LINK A FORM IN WORD TO AN EXCEL DATABASE?

I currently hand write service calls in a purchased service call book and
then enter the information from the service call book to an Excel
spreadsheet. I would like to create a Utility service call form in Word (or
Excel) that can be printed and given to the service call man, and have that
same information automatically be added to a database in Excel so I am not
duplicating entries. Is this possible with these programs?
