Default multipl HLOOKUP/VLOOKUP Criteria

If I can create the correct formula, Table 1 would look like this:

release 1.2.ab dec 1
release 2.3.gh oct 2
release 1.2.ab nov 5
release 2.3.gh dec 6

I do not know if it is possible to next HLOOKUP and VLOOKUP
functions
or if there is a better way to do this.

Any help is much appreciated.
