Default how do i create an excel spreadsheet for NPV IRR & MIRR?

How do I create an excel spreadsheet that calculates the NPV, IRR, & MIRR for
a company? Here is an example of the amounts and percentages that may be used:
an initial investment of $4,000,000,
leases a plant for 5 years,
has an additional other expenses of $750,000 for the 5 years,
a 40% tax rate,
a 12% re-investment rate, and a 10% cost of capital, and
additional hurdle rates of 10%, 15%, & 20% (based on MIRR).
I would appreciate it if someone could direct me to a template to where I
can input the numbers.
