Default how can I type up a consent form

I need to make one because my son is going out of state with his grandmother
and I need to get one made up fast so that if anything goes wrong she can
sign for him at the E.R.

While not an Excel question, probably deserves an answer.

The first one seems simple enough - might want to add line for a notary and
get it notarized.
