Default Spreadsheet of seven types of items

The first item will be the date.
The second item will be purpose in text.
The third, fourth, fifth, and sixth items will be numerical data.
The seventh item will be numerical data.
The third, fouth, fifth items will be totaled at the end of each month, and
again at the end of the year.
If an item is blank, the value should be taken as zero.
