Default saveto or Submit Form functionality in Excel

I would like to create a form that can be completed within our office, that
would have a simple submittal button that could save the answers or the form
in its entirety in a specific area of our network. I would like the name of
the file to be pulled from two fields. I would also like for the person
submitting the form to not be able to see where the form was going. How can
I go about doing this?
