Default LOOKUP function -help!


I have been trying to help my mom with something (she's
taking an online class, but can't figure this out) , I can't even
figure this out! I know it has to be something simple. She's done
everthing herself up till this point, but is totally stuck on this .
This is what she's supposed to do (#3b).

I dont think many people look at attachments, try to explain what you've
done and what you wnat to achieve, you'll get an answer,

b7's formula would be

=vlookup(b5,d5:f17,2)
