This would be the general syntax:

=INDEX(Result_range,MATCH(1,(Lookup_range=Lookup_v alue)*(From_range=From_date)*(To_range<=To_date), 0))

This is an array formula. Array formulas need to be entered using the key
combination of CTRL,SHIFT,ENTER (not just ENTER). Hold down both the CTRL
key and the SHIFT key then hit ENTER.
