Default How do I query a List to generate source data for a chart?

Id like to generate a chart that shows how many claims for each Denial Code
are in the list for each month (columns in List include: Date, Denial Code).
Do you know of a formula to generate a data area, which I could use as the
source data for the graph? The data area Id like would look like:
# of 2006 Denials by Month
CPM Denial Cause Code July Aug Sept Oct Nov
