Default Split mail merge into separate headed columns

I need to format data which is in an address label format into a columned
worksheet with headers. For example:

John Doe
123 Street
Boo, AZ 28288

into:
First Name Last Name Address City State Zip Code
John Doe 123 Street Boo AZ 28288
