Default Print Function? - Combine 2 textboxes and 1 range in footer?

I want to capture the Employee Name (textbox), the Effective Date (textbox),
and the Transaction (range) into the footer when it gets printed. Is there
anyway to do this? Thanks in advance for your help

Linda
