Default Lookup in userform without erasing formula everytime

I have a userform that transfers data to three worksheets. The first
worksheet looks like the userform, but contains information I would like to
print and insert in applicant file, the second worksheet is a summary of all
applicants data, and the third worksheet contains first and last name of
applicant but need to to provide more data later. Problem is that the
userform works, except that it erases the lookup formula after unloading the
userform. I think this is because of the control source property.
