Default Populating a portrait orientated 'form' from Excel rows

A company wants to capture the data on each individual software support
request as individual rows in a spreadsheet, so techs can view all requests
on screen in table view. But they don't want requesters to go directly into
the spreadsheet to make requests or to print out reports on each individual
request.

Instead, they want a separate portrait-oriented 'form' that users can use
two ways: (a) to type in their support requests, and (b) to print out a
record of any individual support request.

Could this all be done witih just one Excel.xls file?
