Default Creating an MSForm based on a table

I read a peice somewhere (book, web, ??) which described how to build
an MSForm at run-time based on a table in a spreadsheet containing all
of the required info (control types, sizes, positions, etc, etc).

Does anybody remember seeing this and have a reference to it?
