Default User Form - Range Selector

Could someone show me some sample code of a user form that has a data range
selector similar to the pivot table wizard and chart wizard?

there really isn't any code to show - you should add the control manually.
You are looking for the Refedit control. It should be in the control
toolbox, or if not, look in the tools menu in the VBE for add more controls.
It should be listed alphabetically as

refedit.ctrl or something very close to that.
