Default user difined inputbox

Hi!
Could someone show me what the code would look like to create and
display a form that the user would enter an alphnumeric string and then
select OK or Cancel and the string would be placed somwhere in the
workbook or the macro would be canceled.
Or possibly there is a collection of examples somewhere I could
download?
Thanks!
