Default Can this be done

I need to create a front sheet for files. when a button is clicked i would
like a series of questions asked and a box to put the answers in so in the
end it all appears on the front sheet.

so if someone clicks create front sheet a box pops up and asks for job ref
with a text box to enter the answer. then on to next question etc.
