Default List with Input Box

Something like Data|form?

If that's close, but not quite, then check out John Walkenbach's enhanced
data|form. You can find it at:

and John's comparisons between his version and excel's at:

Do I have to install the data form add in in evry PC I use the Dataform?
I was hoping that I could design one myself so I have more control over the
form.

I downloaded the Dataform and tried it. I do not think this is the way I
am going.
I just need 2 columns on the form . One for the list of names from the
worksheet and one for the user to put data corresponding to the names and
then export the input to the worksheet.
