Default Can I create a FORM (that I can modify etc) from an existing spreadsheet?

I have a worksheet (imported from a web page) that is set up with the layout
I want.

Is there a way I can import the "look" of this sheet, create a user form,
and then assign the fields to another worksheet (using VBA)
