Default User Form

I've created a macro in Excel that will search for text in
an array and replace it with an underline. The macro
works perfectly except, it would need some kind of user
form since we don't want the user's going into the macro
and typing in the text in the array. i'm not sure where to
begin with the form, it will basically have a text box for
the users to enter the text and then 2 option buttons one
for bold and one for underline after that i'm not sure how
to make everything come together. Any help would be
greatly appreciated.
