Default generic data entry form

Does anyone have 'open' code that is:

a) simple to follow
b) easy to adapt

that will allow data entry to a table that will be created in Excel?
(2000 or XP)

Have looked around, but no code seems too easy to follow or adapt. Am
at intermediate / advanced VBA for excel.
