Default Template for new entries

Hi,

I want to create a template for new entries in a database. Ideally, it
would be one sheet where one could enter the required metrics, which
would automatically be entered in the correct cells on new rows in the
database on the next sheet if a user presses a button or something.
Then, the template would clear itself so a new entry can be made. Can
someone tell me whether this is possible or not, and if so, how I could
build this?

Thanks in advance,

Peter
