Default Helpl with formula lookup

Dear Friends.

Could you please suggest a lookup formula for the attached file ? Thanks heaps

The formula will need to reference Rows 1,3,5,7,9,11,13

in B2:
=INDEX('Look UP Data'!$A$1:$AW$2,2,MATCH(A2,'Look UP Data'!$A$1:$AW$1,0))
in C2:
=INDEX('Look UP Data'!$A$3:$AW$4,2,MATCH(A2,'Look UP Data'!$A$3:$AW$3,0))

and copy both formulas down and format your date
