Default Vlookup using two criteria -not working

Hello,

Could you please refer the attached worksheet and kindly suggest a formula to look up values using two criterias.

I used index, match functions and they dont seem to work.

Please help

in C3 try:
=INDEX('Look UP Data'!D$1:D$100,MATCH($B$2&$B3,'Look UP Data'!$C$1:$C$100&'Look UP Data'!$A$1:$A$100,0))
and enter the array formula with CTRL+Shift+Enter
Then copy the formula to right and down.
