Default Look up with multiple variables

Hello Excel gurus,

this may sound simple to you. Could you please help me to find a formula to look up values using two variables as shown in the attached spreadsheet ?

try in E2:
=INDEX($J$4:$J$16,MATCH(A2&C2,$H$4:$H$16&$I$4:$I$1 6,0))
and enter the array formula with CRTL+Shift+Enter
