Default Excel not printing entire range when scaling

I am using Excel 2007 and I have established the following Page Setup settings:

Orientation: Landscape
Scaling Fit to: {blank!} page(s) wide by 2 tall
Print area: D6:EE148
Rows to repeat at top: $1:$5
Columns to repeat at left: $A:$C

When I go to print, Excel scales and prints the range D6:DV148. I do not
understand why Excel does not print the remaining 9 columns, or how to
correct this problem.
