Default insert a file

I have a form created that is protected and what I am trying to do:
I want to be able to open a copy of the form , fill out the information and
then save with a report number name. I have a macro that when I open the
report named file and run the macro, it unprotects the document, but, how do
you add a blank copy of the original form? I want to be able to keep reports
together. If someone could tell me how to add a blank copy at the end, then
I could create the macro to unprotect add the blank copy and then protect the
form. Thanks,
