Default Tiered Pull Down Boxes

I have a 4 level deep pull down box situation (Choose Level 1, Options for
level two are based on level 1, etc....)

This is being used in a form where this is being used for multiple order
lines. { Sports, Baseball, Glove, Right handed} So the same question can get
asked 150 times since people can order multiple items.

Is there a simple way to accomplish this, or do I need to use a database?

Maybe you could use Data|Validation and use dependent lists.
