Default Health record Template

I am looking for a template to use to record the results of recurring blood
and other medical tests that are taken periodically and frequently so that I
have records of all readings (different categories) over a period, eg
cholesterol, blood sugar etc.

you are asking for a fairly specific template. microsoft has templates for
everthing(i think). see this site. microsoft template home. snoop around.

if you don't find anthing pre-made, then consider the microsoft template
wizard.

i have never used this wizard. per microsoft, good for 2003 & back + 2007.
